SURGERY TO OPEN MAXILLARY SINUS TO REMOVE TUM AND EXTRACT TEETH IN THE SINUS

On October 4, 2023, doctors from the Department of Ear-Nose-Throat (ENT) and Dentistry-Maxillofacial at Thai Nguyen International Hospital cooperated to perform surgery to open the maxillary sinus to remove the tumor and extract tooth in the sinus for patient B.N.A, 31 years old, Chua Hang ward, Thai Nguyen city.
Recently, Ms.A had a stuffy nose and headache. Taking medicine did not help. The stuffy nose and headache gradually increased, so she went to Thai Nguyen International Hospital for examination. The patient was examined endoscopically, and the doctor concluded that he suspected a tumor in the left maxillary sinus. Patients were assigned CLS to confirm. The results of CT scan and magnetic resonance imaging (MRI) of the sinus system showed: The lesion in the left maxillary sinus, measuring 37x51mm, destroyed the sinus wall and invaded the left ethmoid sinus, pushing and shifting the nasal septum, there are also hidden teeth in the sinuses.

After an interdisciplinary consultation with ENT and Maxillofacial specialists, the doctors concluded: The patient has a left maxillary sinus tumor with teeth growing underground in the sinus that has destroyed the wall of the maxillary sinus, which can affect the near tissues. Diagnosis conclusion: Dental cyst. This is a rare medical case with bone destruction and requires early surgery to avoid dangerous complications. The patient was prescribed surgery to open the maxillary sinus to remove the tumor combined with the extraction of hidden teeth in the sinus. The surgery was performed simultaneously by two surgical teams: Ear-Nose-Throat and Dentistry-Maxillofacial specialists. Determining that this was a complex surgery, the surgical team chose to operate in two ways, combining endoscopic sinus surgery and opening the left maxillary sinus according to the Caldwell-Luc route to expand the surgical field to perform both at the same time to save medical costs and treatment time.

The surgery was successfully performed, removing the entire cyst and removing all the hidden teeth in the left maxillary sinus. After the post-operative treatment period, the patient’s current health condition: stable, no stuffy nose, no headaches, no discomfort and very satisfied with the results of the surgery.
