SUCCESSFULLY RESCUE A PATIENT WITH MULTIPLE INJURY SHOCK BECAUSE OF A TRAFFIC ACCIDENT

 Recently, doctors from the Department of General Surgery, Yen Binh Thai Nguyen General Hospital, performed emergency surgery on patient N.T.V, 44 years old, Pho Yen City, Thai Nguyen province, who suffered multiple injuries from a traffic accident.

Ms. V had a traffic accident and was taken to Yen Binh Thai Nguyen General Hospital for emergency treatment in a situation of shock and multiple injuries. Immediately, doctors resuscitated and performed tests, ultrasound, X-rays, and computed tomography to determine the patient’s condition. The results showed: ruptured spleen, fractured posterior arch of left ribs 3, 4, 5, 8, fractured middle 1/3 of left clavicle, fractured left scapula.

The hospital conducted an emergency consultation, the doctors concluded: Ms. V had traumatic shock, grade 5 spleen rupture, left rib fractures 3, 4, 5, 8, left clavicle fracture, left scapula fracture, was prescribed emergency surgery to remove the spleen to stop the bleeding. The patient was actively resuscitated with a transfusion of 6 units of blood, and surgery was performed to stop the bleeding. On the 3rd day after surgery, the patient had difficulty breathing, chest pain, after having X-ray, the result showed: Hematoma in the left pleural cavity was severe, and a closed drainage of the pleural cavity was prescribed, draining nearly 1000ml of blood in the pleural cavity, drainage was continuously suctioned under a pressure of 20cmH20 for 3 days. On the 9th day, Ms. V was scheduled to have surgery to fuse her left clavicle with screws. After 2 weeks of intensive treatment, the patient’s overall condition was better, the surgical scar was healed. Ms. V had no difficulty breathing, was stable and was discharged from the hospital.
