The Department of Obstetrics and Gynecology at TNH Pho Yen Hospital has successfully treated a rare and complex gynecological case using laparoscopic total hysterectomy, thereby affirming the high level of professional expertise and mastery of advanced surgical techniques of the hospital’s medical team.
The patient, a 41-year-old woman with a history of two vaginal deliveries, had noticed a long, firm mass accompanied by inflammatory discharge in the vaginal area over the past four years, causing significant discomfort and disruption to her daily life.
Upon direct examination, Dr. Hoang Thi Kim Ngan, MD, PhD – Head of the Department of Obstetrics and Gynecology at TNH Pho Yen Hospital, observed that the patient’s cervix was severely hypertrophied, measuring up to 8 cm in diameter, with inflammation, an irregular surface, and easy bleeding of the external cervix. Fortunately, diagnostic evaluations and pathological test results confirmed that all lesions were benign, with no signs of bladder or rectal prolapse.
Based on multidisciplinary consultation, comprehensive clinical assessment, and the patient’s desire—having completed childbearing and with no further reproductive needs—the medical team decided to proceed with laparoscopic total hysterectomy. According to the optimal treatment protocol, both ovaries were preserved to maintain normal female endocrine function, thereby contributing to improved quality of life and psychological well-being after surgery.
The surgery was performed by an experienced surgical team led by Dr. Nguyen Van Tu, MD, PhD – Director of TNH Pho Yen Hospital, together with Dr. Hoang Thi Kim Ngan, MD, PhD – Head of the Department of Obstetrics and Gynecology, in coordination with the Department of Anesthesiology and Resuscitation.
According to the physicians, the major challenge and rarity of this case lay in the markedly enlarged and firm cervix, which posed significant technical difficulties during the surgical procedure. However, thanks to the team’s extensive expertise, close collaboration, and proficiency in laparoscopic techniques, the surgery was completed safely and successfully, with effective hemostasis and adherence to all professional standards. Following the hysterectomy, vaginal reconstruction was performed to preserve anatomical structure and ensure normal functional outcomes for the patient.
