Declaration of Eligibility for Fixed Vaccination Services of the Vaccination Center – Thai Nguyen International Hospital

Pursuant to Decree No. 104/2016/ND-CP dated July 1, 2016 of the Government regulating vaccination activities; Decree No. 155/2018/ND-CP dated November 12, 2018 amending and supplementing several regulations related to business investment conditions under the management scope of the Ministry of Health; and Decree No. 13/2024/ND-CP dated February 5, 2024 amending and supplementing several articles of Decree No. 104/2016/ND-CP dated July 1, 2016 regulating vaccination activities;

Pursuant to Circular No. 34/2018/TT-BYT dated November 12, 2018 of the Ministry of Health detailing several articles of Decree No. 104/2016/ND-CP dated July 1, 2016 on vaccination activities;

Pursuant to Notice No. 2127/TB-BVQTTN dated December 26, 2025 of Thai Nguyen International Hospital regarding the notification of eligibility for vaccination services;

The Thai Nguyen Department of Health hereby republishes and posts on its official electronic information portal the declaration of eligibility for fixed vaccination services of Thai Nguyen International Hospital as follows:

Facility Name: Thai Nguyen International Hospital.

Address: No. 328 Luong Ngoc Quyen Street, Phan Dinh Phung Ward, Thai Nguyen Province.

Head of Facility: Assoc. Prof. Dr. Pham Thi Hong Van.

Person in Charge of Vaccination: Specialist Level I Doctor La Thi Ngoc Nga.

This announcement takes effect from the date of signing and replaces all previous published declarations regarding vaccination eligibility for Thai Nguyen International Hospital.

Thai Nguyen International Hospital shall take full legal responsibility for the declared vaccination eligibility documents and shall comply with all regulations on vaccination activities in accordance with Decree No. 104/2016/ND-CP dated July 1, 2016 of the Government and other applicable legal regulations./.
