EMERGENCY TREATMENT OF A TRAFFIC ACCIDENT PATIENT WITH GRADE III LIVER RUPTURE

On December 13, 2025, TNH Pho Yen Hospital admitted patient Ngo Thi Thu Ha (30 years old) in critical condition following a traffic accident. Upon admission, the patient presented with a rapid heart rate of over 120 beats per minute, hypotension, pale skin and mucous membranes, severe abdominal pain, and an agitated mental state. Initial assessment confirmed a Grade III liver rupture with massive intra-abdominal bleeding—a serious and life-threatening injury requiring immediate surgical intervention.

Immediately after admission, the hospital’s trauma emergency protocol was urgently activated. The patient simultaneously underwent abdominal ultrasound, contrast-enhanced abdominal CT scan, and comprehensive hematological, biochemical, and coagulation tests. Imaging results revealed a laceration of the liver parenchyma in subsegment VII measuring approximately 56 × 38 mm, with significantly elevated liver enzymes due to trauma. Coagulation parameters and renal function remained within controllable limits, providing favorable conditions for specialized surgical management.

Emergency surgery was promptly indicated, with close coordination among the General Surgery, Emergency & Intensive Care, and Anesthesiology & Resuscitation departments. Throughout the treatment process, vital signs, blood loss, liver enzyme levels, and inflammatory response were continuously monitored. Postoperatively, the patient was conscious and stable, with a marked decrease in liver enzymes and hematological and coagulation parameters maintained within safe ranges. The patient responded well to the treatment regimen and met the criteria for discharge.
